How Our Referral Process Works

Our Admissions and Clinical Partnership Teams play important roles in communicating with you, your client, and others involved in their care including family members. Communication is key so we provide a concierge level of service by answering questions and guiding everyone through each step efficiently, ensuring the appropriate level of detail.

To begin the process, either reach out to your Clinical Partnership Team representative, call our Admissions Team directly at 419-885-8800, or fill out the form. Our initial call will focus on gathering key information and answering questions before moving into the pre-admission assessments.


During the pre-admission process, our team will work with you to gather specific details we will utilize to determine clinical alignment as well as the required level of care. Financial options for your client and a benefit assessment will also be provided during this process. We each client where they’re at and work to instill a sense of urgency to help them receive treatment and relief as soon as possible.

At Admission

We know your client’s wellbeing is your primary concern. We are committed to partnering with you to keep you informed with regular status updates. We will notify you when your client has arrived safely and has begun the intake process.

Levels of Care

At Toledo Center, we offer a range of intensity, including residential treatment and partial hospitalization treatment to help support your client’s unique clinical needs on their road to recovery. We will work closely with you and your client find the best fit within our programs.
Adolescent Residential Treatment (RTC)
Offering a high level of intensity, structure, safety, and monitoring for adolescents of all genders (ages 12-17), our RTC program provides a comfortable living area with shared rooms and a common living space where clients receive care 24 hours a day, seven days a week.
Adult Partial Hospitalization Program (PHP)
Our PHP provides evidence-based treatment balanced with activities aimed at enhancing mindfulness, encouraging healing, and helping with the development of new modes of self-expression.
